If your passport expired more than 5 years ago, then you will need to appear at a passport acceptance facility to have your identity verified and pay $35 for the “execution fee” in addition to government fee listed above. If your passport was issued less than one year ago, you can change the name in your passport without paying a fee. (However, if you choose the expedited service, there is an additional charge.)
