Applicant lost the passport.

Applicant lost the passport and the passport was expired less than 5 years ago, OR they are unsure.

Applicant is in possession of one of the following:
- A long form certified birth certificate issued by the vital records office from the state they were born, OR
- Certified birth record for the applicant if he or she was born abroad, OR
- Naturalization Certificate, OR
- An older, undamaged cancelled passport by the U.S. Passport Agency.
- If the applicant is under the age of 16 years old (see the requirements for Child Passport), OR
- If the applicant is above the age of 18 years old (see the requirements for Adult Passport).

The form must be filled out online and printed as a PDF file on single sided paper. This link will direct you to the passport application: https://pptform.state.gov/. Please make 2 copies.

For the option to choose the products and fees at the end of the form, applicant must choose the passport book ($110.00) and expedite fee ($60.00) = $170.00. If the applicant is under the age of 16 years and needs a passport book only, choose ($80.00) and expedited fee ($60.00) = $140.00 (a personal check, money order or a cashier's check from a bank must be addressed to the US DEPARTMENT OF STATE), plus a $35.00 (cash, check, or money order) for the Authorized Passport Application Acceptance Facility. Applicant must report the lost or stolen passport by filling out form DS-64 (if the lost or stolen passport was issued more than 15 years ago, the DS-64 is not required).

Reminder, the applicant must indicate that his or her passport is lost or stolen when filling out
the DS-11 form. -

Sign and date it if the applicant 18 years old or above, OR -
The applicant is 16 or 17 years old, he or she must sign and date the DS-64 and one of the parents must sign, OR

The applicant is under the age of 16 years old, both parents or guardians must sign and date the DS-64.

Do NOT sign this form until the applicant is verified by the Authorized Passport Application Acceptance Facility.

Two original copies of the Letter of Authorization. This letter must be filled out by the applicant with a pen (Black or Blue) as follows:
- Please check the first and second box only.
- Then put Last, First, Middle name.
- Applicant phone number as show in the printed application.
- Date must be the date you filled out the form, MM/DD/YYYY.
- Courier Company Name MUST MATCH THE LETTERHEAD as shown on the top of the letter of authorization.
Please DO NOT put FEDEX or UPS, this may delay the process.
- Applicant Signature, if the applicant 16 years old and under the age of 18,he or she Must sign the letter of authorization with 1 of the parents, if the applicant under the Age of 16, both parents or legal guardian must sign the letter of authorization.

- A travel itinerary must be included with your documents or a letter from employer with the letterhead of the company addressed to the Passport agency requesting an expedited
1 Week Service Fee $400
2 Weeks Service Fee $300
3 Weeks Service Fee $120
REMINDER: Applicants must also pay the government $190 passport book fee for the 1 Week Service (by personal check, money order or a cashier's check from a bank) addressed to the US DEPARTMENT OF STATE (as mentioned above in Guideline for filling the application). The government fee for the 2 Week and 7-9 Week Service is $209.53. Just choose the appropriate service option below.
